U.S. News & World Report Names Swan Creek Among the Top Nursing Homes Nationally

Swan Creek Retirement Village, 5916 Cresthaven Lane, Toledo, has been named one of the nation’s top nursing homes by U.S. News & World Report in its February edition. An Ohio Presbyterian Retirement Services (OPRS) community, Swan Creek Nursing Center was among the top eight nursing homes in Ohio and 173 throughout the nation.

The U.S. News rankings rely on Nursing Home Compare, a program run by the federal Centers for Medicare and Medicaid Services. CMS analyzes information on all homes enrolled in Medicare or Medicaid (other than about 150 too new to provide data) and gives them ratings of one to five stars overall. The homes also receive ratings of one to five stars in each of three areas: health inspections, nurse staffing and measures of care.

“Swan Creek’s five-star status is well deserved and shows that OPRS strives for excellence and the well being of our residents,” OPRS Chief Operating Officer Daniel J. O’Connor said. “Congratulations to Jean Morgan, Swan Creek executive director, and the fine staff for achieving this recognition!”

Swan Creek is a not-for-profit continuing care retirement community serving the Toledo area since 1992. OPRS is the largest and most experienced not-for-profit provider of continuing care retirement communities and services in Ohio. A faith-based organization with headquarters in Columbus, OPRS serves more than 80,000 people in 42 Ohio counties annually through its 11 retirement communities and its Senior Independence home and community based services division.
